In “Point of Return”, Dean carefully choses his words to hit both Bobby and Sam where it hurts most. He tells Bobby, who has been more of a parent to him and Sam than John ever was, that he has no right to tell him anything because isn’t his father. Sam, who’s been struggling with his conscience and guilt all year, he tells that he’s not strong enough and falls for everything with the right bait.
He’s being a dick to both of them by choosing insults that are both unfair and respond to their insecurities.
But what does he say to Castiel? He chooses sexual comments. Does that mean he knows about Castiel’s attraction to him? That he’s trying to get to Castiel the same way he does with Sam and Bobby, but in Castiel’s case by reducing their relationship like this?

Well, maybe not KNOWS about Cas’ attraction. I think Dean’s been dancing around that for ages because he’s scared to actually acknowledge Cas’ feelings for him and the fact that they might be romantic and/or sexual. Scared because he doesn’t think he deserves that kind of love, because he doesn’t think he can reciprocate, because he thinks he can and/or does reciprocate - pick one.
But yeah, this was fuel for the Dean/Cas fire like WHOA :)
Wow what if Benny was the one who cut Cas off the ladder out of purgatory, and Dean looks back to see his friend being ‘killed’, but Benny lets Dean believe it was an accident or it was Cas’ choice to stay. And then Dean is haunted by the flashbacks of his friend ‘dying’ for ages until Cas comes back to let him know Benny aint a bro.
