never mistake that Dean turns around because future!dean wasn’t pointing at him. The camera fucking goes from blurry to clear and they show a Dean reaction shot. That is the exact moment when Dean found out that he and Cas in the future had a sexual relationship. He probably had high suspicions when he saw Cas but that (and the conversation that follows) seals it for him.
That is why he he says “send you friends to die…and Cas too” because he knew that Cas was and has always been more than a friend and he for sure knew that he was more than a friend to future!dean.

Sam is possesed

Dean is alone, but Cas is there with him

Dean watched as his future self walked across the room, picking up his gun and securing it in its holster. Dean let’s his eyes fall down to trace the two straps firmly wrapped around the other Dean’s thigh.
“I bet chicks really dig that thing.”
Future Dean stops in his tracks, looks down then back up at Dean. “This thing?” His finger lightly trails across the top strap. “Well, I suppose.” He walks towards Dean with a crooked grin.
Dean can feel the heat creeping up his neck with each step. Swallowing the lump in his throat, his eyes flick up and down to the thigh holster then to his future self’s eyes.
“But they’re not the only ones.”
Dean shivers when that thigh is pressed between his legs.
“When Jensen runs, even our stunt guys can’t catch him. He’s like ‘I’m only going half speed!’ and the guys are fading in the distance,” says Lou Bollo [Supernatural stunt coordinator].

I mean who the hell do you need to be banging to get a cabin like that at the literal end of the world? (oh wait hang on we know the answer to that.) can’t you just picture it, though? I wonder if it’s guilt-indulgence from dean’s side, letting cas surround himself with all this stuff when you know he’d give anyone else in the camp a reaming for it. distractions and baubles, not unlike the sex and the drugs, things to fill up the spaces that grace leaves behind?

Whatever the Cost:
Cas shrugs. “There’s a chance you’ll survive, you know.”
“Slim.” Castiel turns to Cas, grasping for other options. “You’ve had four extra years. Is there nothing else? No other way?”
Cas shakes his head. “Nothing.”
