Can we talk about how powerful and incredible that line is?

Already Cain’s acknowledging that “that one” is a big one. That Castiel is an important and predominant figure in Dean’s life. That one. A whole separate sentence just to put emphasis on his point.

What Cain suspect is that Dean loves Cas the way he loved Colette. He suspects it because it’s not shown by Dean and Cas the same way it was for him and Colette, not yet anyway. The best we got was Dean including Cas when he said “the people who love me.” We don’t know what Cain knows exactly, what he’s heard, what he’s seen, but he knows enough to know that Cas is special. And he thinks he knows Dean, he talks like he knows Dean’s life and Dean’s heart. And he wants to prove it to him with every word. “Even though you never said it, I suspect that you love him.” That’s what he’s saying. “And killing the person you love, it hurts something awful. I know that.“ And he’s also saying ”I know you, maybe even better than you do. I see you and him, and I see myself in you.“
Killing Cas, Cain knows, will be a special kind of pain for Dean because he remembers how it felt to kill Colette. Cain truly believes that Cas is Dean’s Colette, that he’s his redemption, his savior, his lover, and that it will hurt something awful when he has to face his fate with him.
But I do think he’s underestimating the nature and power of the bond between Dean and Cas. And I just can’t wait to see how it unfolds.
