I know people keep talking about how sad it is that Dean didn’t want to be fixed up because he felt guilt and this was his “penance” so to say but

it’s also very forgiving. 

cas is never forgiven even for things that are not directly his fault. Remember the last time he was being controlled and almost killed Dean? Dean gave him the cold shoulder and told Sam if anyone ever tried that before, he would kill them on principle. 

But I think the whole MOC thing made Dean understand that certain things will make you do things beyond your control even if you don’t want to do them and in the end makes you feel like a terrible person and wracks you with guilt and wanting to make things better.  

i think it was the first time Dean actually understood what the hell Cas was going through. 

It was basic understanding. and for once Cas may have felt guilty on his own for doing it but he was met with someone who didn’t blame him and meant it.
